Critical Care Consult Request

Critical Care Consults are available for ultrasounds and/or radiographs Monday through Friday from 9a-5p MST/MDT. Veterinary staff, please fill out the form below to request a same day critical care consult.

Once your patient’s diagnostics are reviewed by our team and a report has been generated, our Criticalist, Dr. Megan Tzipory, will reach out the same day, via phone, to discuss the case directly with the primary veterinarian. Consultation includes review of patient records, discussion with primary veterinarian and recommendations for the medical plan. A summary letter of this consultation will be emailed to the requesting veterinarian within 24hrs. Please ensure you email your patient’s pertinent records/diagnostics/blood work to Contact@EclipseVetImaging.com when submitting a request.
